AXA Equitable Life Insurance Beneficiary Change Form

The AXA Equitable Life Insurance Beneficiary Change Form is used to update the beneficiary information for an AXA Equitable life insurance policy. The form can be completed online, by phone, or by mail.

To complete the form online, you will need to have your policy number and the following information about each beneficiary:

• Name
• Date of birth
• Social Security number or tax ID number
• Relationship to the insured
• Percentage of benefits to be received

You can also complete the form by phone by calling AXA Equitable customer service at 1-800-444-4194. To complete the form by mail, you can print it out from the AXA Equitable website and mail it to the following address:

Important Information about Beneficiary Designations

• You may name anyone or any entity as your beneficiary.
• You may change your beneficiary at any time by completing a new Beneficiary Designation/Change form.
• If you do not name a beneficiary, the proceeds of your policy will be paid out according to the terms of your policy.
